SUMMARY: Ability360 is a nonprofit organization that offers and promotes programs designed to empower people with disabilities to take personal responsibility so that they may achieve or continue independent lifestyles within the community.

Coordinate Community Development Block Grant (CDBG) and Arizona Long Term Care System (ALTCS) Home Modification projects and perform assigned responsibilities in the Community Integration Unit. Work closely with City CDBG contract monitors & ALTCS Case Managers & their Supervisors, consumers and contractors to achieve home accessibility. Monitor contract compliance. Work with individuals with disabilities to assess their functional ability and determine solutions that will increase access, safety, and independence at home. Complete assigned responsibilities with emphasis on assessing consumer needs, developing goals, and providing a safer home environment.

SUPERVISOR: Vice President of Community Integration

FSLA STATUS: Exempt

RESPONSIBILITIES: (with or without reasonable accommodations)

  • Conduct eligibility process, including pre-screenings
  • Coordinate home visits with Ability360's Home Modification Program Assessor to: (1) Conduct comprehensive in-home and environmental assessments (pre-and post-service) of consumers' functional ability at home; (2) Obtain necessary eligibility documents (per funding source)
  • Assist consumers in determining their home access needs and establishing related independent living goals/ plans
  • Coordinate lead-based paint visual inspections
  • Assist in developing scopes of work
  • Write report, findings, recommendations and documentation for both Environmental Reviews and for submission to funders (as required)
  • After reports are approved, coordinate bids and select awarded contractors
  • Coordinate construction work with consumers and contractors to ensure it meets specifications/consumers' needs and verify all necessary permits
  • Provide accurate information and appropriate referrals to other Ability360 programs/ services and community resources as appropriate
  • Once completed, submit all billing information to Ability360's Finance Department
  • Maintain documentation consistent with Ability360's policies and procedures and funding requirements (including assessment, goal planning, information and referral, output/ outcome measurements, etc.)
  • Ensure all documentation is completed and downloaded in to Ability360's database system.
  • Participate in activities advocating systems change for people with disabilities - particularly opportunities to live in their own homes and communities in lieu of costly, unnecessary institutionalization
  • Work collaboratively with Ability360's program team, community organizations, and governmental entities to promote independent living and disability rights
  • Maintain knowledge of current trends and developments in the field through review of literature, attendance at trainings, and networking
  • Be responsible for the timely conclusion of all home modification projects

MINIMUM QUALIFICATIONS:

  • Understanding of, and passion for, Ability360's mission and vision
  • Experience working with individuals with disabilities
  • Working knowledge of accessibility standards/ laws (ADAAG and/ or ANSII-ICC A117.1) and disability and aging issues (housing rehabilitation, aging in place modifications)
  • National home access/modification certification - Certified Environmental Access Consultant (CEAC), Certified Aging-in Place Specialist (CAPS), and/ or Certified Accessibility Inspector/ Plans Examiner
  • Proficiency with MS Office as well as database and assessment software
  • Excellent customer service, interview, and communication skills
  • Subject to criminal background check prior to employment (AZ Level 1 fingerprint clearance required) and proof of COVID-19 vaccination

Work Environment:

Position requires work inside the office or in a home office.

Position Type/Expected Hours of Work:

This is a full time salaried position. Days and hours of work are typically Monday through Friday, 8:00 am to 5:00 pm.
